Prize Winners Must Fill Out Online W9 Form

ALL U.S. residents who won prizes in the NFBKC this season must fill out the Online W9 form on your team page. The only exceptions are for anyone who has won prize money with us in any of our contests the past three years and filled out the Online W9 form previously.
