Shared Prize Forms Due April 10

Shared prize forms are available again this year. These forms must be notarized and emailed back to me by Wednesday, April 10. If you would like a form email me at [email protected] to receive one.
NOTE: Anyone receiving prize payouts MUST have a player account in order for the check to be sent to them. So if you have a co-owner who does not have an account, have them go to playnfbkc.com and create one so they are all set. Thanks.
